We envision some who is an experienced, highly skilled Registered Nurse that shares our passion providing personalized and attentive medical care to patients in their homes in the Madison area. Job Responsibilities Complete nursing assessment, monitoring, evaluation, interventions, and treatment to care for acute care patient populations in the home. Apply nursing knowledge and critical thinking skills to identify, prevent, and solve complex acute patient care issues. Collaborate with the patient, family, interdisciplinary team, therapists, specialists, and utilize community resources to provide effective care coordination. Evaluate social determinants of health and incorporate into the patient’s individualized care plan and care management. Assist with biometric monitoring technology and respond to biometric vital sign data. Provide individualized patient education on disease and symptom management, medication management, treatments, procedures, and therapies. Share pertinent assessment findings with effective communication skills to collaborate with the remote provider and multidisciplinary team members. Prepare for and coordinate discharge from hospital at home with patient and family to ensure seamless transition to patient’s PCP, home care services, and specialists, with community resources as appropriate. Ensure all interventions in the patient’s care plan are implemented with a patient centered approach. Competence in procedural skills including, but not limited to, phlebotomy, vascular access device management, hydration therapy, medication administration (oral, intravenous, intramuscular, rectal), wound management, catheter care (urinary, pleural, peritoneal), specimen collection, stoma management, and compression therapy. Maintain a therapeutic and professional patient-nurse relationship. Assist with program development of the RN role in providing nursing care and care management in the patient’s home. Serve as a resource in quality improvement processes and technique. Maintain and practice in accordance with nursing guidelines per state and federal regulations. Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents. Participate in quality and safety investigations and root cause analyses. Take appropriate action to ensure patient safety as a result of any investigation. Other duties and responsibilities may be assigned. He / she will bring : Registered Nurse (RN) – active state licensure (WI) or ability to obtain by start date Associate’s / Bachelor’s Degree in Nursing from an accredited nurse program 2 years of nursing experience Previous experience in acute / ICU / ER / home health setting highly preferred CPR / BLS certification Valid driver’s license Excellent interpersonal, (verbal / written) communication, and organizational skills Knowledge of medical terminology, procedures, and treatments High clinical proficiency in nursing Proficient in technology including EMR systems and Windows based computer skills Knowledge of state and federal home health regulations Ability to exercise independent judgment and initiative Individual compensation is based on skill set, experience, qualifications, location, labor markets, and other job-related reasons.
