Purpose Statement:
The role of the Designated Coordinator In Training is created to allow individuals to be better positioned to take on a larger leadership role as Designated Coordinator in an existing site or new site as opportunities become available.
Job Duties and Responsibilities
Maintain a clean, safe, and healthy environment with light housekeeping, including but not limited to, laundry duties- Promote independence, not dependence with the person served offering choices and fostering self-help skills
- Report any changes in the person served condition using data collection technology and Designated Manager in a timely manner
- Complete Incident Reports according to policy
- Reporting directly to the person served and their external team on the weekends when scheduled an On-Call rotation
- Clocking in and out utilizing EVV technology; including duties completed in shift notes and obtaining timesheet signatures after each shift.
- Complete mandatory online annual trainings modules -assigned and due monthly
- Topics include: Activities of Daily living, Infection Control, Vulnerable Adult, Abuse Prevention, HIPPA, Bill of Rights, and Healthy Diets
- Encourage, gather and/ or transport person served to participate in scheduled activity programs
- Socialize with person served and offer new opportunities to incorporate into their lives
- Develop and maintain a positive working relationship with DC, DM, and other ILSCM staff
- Communicate information regarding person served and potential changes to care plan to internal team
- Be compassionate and work with tact and ethical awareness
- Be self-sufficient
- Be flexible and adaptable to changing situations and client schedule needs
- Ability to remain calm in difficult or unusual circumstances
- Locating and utilizing necessary documents to complete shift assignments (i.e. address, phone number, program documentation)
- Responds appropriately to safety hazards, fire, weather, or emergency situations
- Follow through promptly on requested duties
- Must be mature and emotionally stable using a calm and kind tone of voice
- Support the mission and values of ILSCM
Other Duties as assigned or scheduled:
- Complete client paperwork as assigned by a DC or DM
o Intake paperwork, annual, Initial Planning Meeting, Person Specifics, Person Centered, Case notes, and other 245D paperwork
- As directed by a DC or DM, communicate with person served team communication
o Case workers, social workers, family members, and others involved in the care team
- Prioritize and organize work effectively and efficiently
- Learn about 245D licensing regulations under DC supervision
- Incident Reporting review and education under DC supervision
Required Skills and Experience (Minimum requirements in terms of educational background, work experience, licenses/certifications or other knowledge skills and abilities)
- Must have a valid drivers license, if driving is required
- Ability to pass a background check with the Department of Health and Human Services
- Possess skills to communicate effectively with person served, families, staff and other customers
- Possess the ability to read, write and comprehend simple instructions, correspondence and documentation, including medical terminology, in English
- Be punctual with a good attendance record
- Must demonstrate good interpersonal skills
- The designated coordinator must minimally have:
(1) a baccalaureate degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children;
(2) an associate degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children;
(3) a diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children; or
(4) a minimum of 50 hours of education and training related to human services and disabilities; and
(5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
Additional Skills and Experience (Preferred or helpful)
- Experience working in 245D or healthcare
The pay range for this role is:
19 - 21 USD per hour(ILSCM-Sartell)
