Type: Tribal Salary Range: $0 to $0 / Per Year Open Period: 1/19/2015 to 2/19/2015 Summary: The Diabetes Program Coordinator is responsible for administering the SDPI (Diabetes) grant and facilitating accomplishments of the goals and objectives within the grant. Will work with clientele and youth on diabetes prevention, maintenance and related objectives. Will work with Nez Perce and other Tribal community members. Will work with Tribal, clinic and school programs on activities and events for diabetes prevention. Provide individual patient assessment, care education, care plan and documentation; evaluate outcomes; provide diabetes/diabetes prevention education, including individual, group or in patients home if it would be hardship for the patient to come to the clinic and/or Support Group. Plan, develop, promote and facilitate continuing medical education for health care professionals of all disciplines in the care and treatment of diabetic and pre-diabetic patients; assist with development of individual staff members seeking CDE; chair and provide monthly reports to the Diabetic Task Force; research and develop funding alternatives to enhance program. Enhance and maintain diabetes and pre-diabetes registries; provide annual diabetes audit to Portland Area Office; track all patients registered at NMPH with diabetes for completeness of care (meet ADA Standards of Care); assist in development of clinical pathways/standards as requested; and develop and distribute educational materials targeting particular issues and particular age groups. Facilitate Access to fitness programs in communities of heaviest concentration of NMPH clients. Fitness resources will include physical facilities and equipment, programs, classes and consultations. Monitor use and evaluate effectiveness of fitness programs; facilitate NMPH fitness equipment maintenance. Supervise and assist with diabetes/pre-diabetes/ diabetes prevention classes and programs. Maintain policies, procedures and records associated with the diabetes program; including fitness center policies and procedure; education policies and record keeping; diabetes and pre-diabetes registries and other grant-related data and reports. Participate in Diabetes Care Team to assist with application of current standards of care to patients with diabetes and pre-diabetes; assist in case management as needed; participate in diabetes clinics; work with Foot Care Nurse, Podiatry, Dental, Optometry, and Contract Health Services to facilitate patient assistance with diabetes-related special needs as outlined in the SDPI grant. Other related duties as assigned. Qualifications: Requires a Degree in Healthcare Education or relevant field with a working knowledge of Diabetes. BSN or MSN preferred. Must have experience with program management and grants program management, including skills in program coordination, budgeting, reporting and supervision of staff. Requires certification as a Diabetes Educator. Prefer experience in business applications and marketing; also the ability to facilitate and lead work groups. Must be able to work independently, handle conflict and crisis in a professional manner. Must be computer literate, have excellent communication skills, both oral and written. Subject to a background check and pre-employment drug screening. Requires driver’s license and an insurable record. Tribal preference applies.
