- Care Coordination Manager
- to join our team. In this position, you will be responsible for the day-to-day case management operations of each care center and oversee interdisciplinary rounds as needed. You will also provide staff mentoring and professional development, which includes recognizing and counseling all case management staff.

Responsibilities :Track and monitor outcomes, including length of stay, readmission, denials, patient satisfaction and patient rights itemsAssist in tracking and reporting patient rights metrics, including patient preference, Medicare messages and Medicare outpatient observation noticesServe as a resource for case managers and other interdisciplinary staffMonitor staff attendance, timesheets and PTO requestsMonitor and oversee new staff orientation and evaluationComplete yearly evaluations for case management staffMaintain clinical competency and current knowledge of regulatory and payor requirementsProvide rotational on call services for the Care Coordination Department (Sat and Sun, 8AM – 4 : 30PM)Qualifications :BSN required; MSN preferredRN license in New Jersey; Case Management certification preferred10 years of healthcare experience within the last 15 years; Management experience preferred5 years of case management and utilization review experience within the last 7 yearsKnowledge of MCG criteria and understanding of Centers for Medicare & Medicaid Services (CMS) guidelinesUnderstanding of revenue cycles and UM processesCapability to work as part of a team to improve care transition and overall patient experienceStrong verbal, written and interpersonal communication skillsWeekend on call rotation is requiredAt Inspira Healthyou’ll join the area’s most dedicated and distinguished team to bring quality and compassionate care to our communities.
