Job Title: 24-Hour Homeless Shelter Case Manager
Job Summary:
The Case Manager will work in a 24-hour homeless shelter providing direct services, case management, and resources for individuals experiencing homelessness. This role involves assisting clients with immediate needs, developing long-term plans for housing stability, and connecting them with necessary community resources and services.
Key Responsibilities:
- Intake and Assessment:
- conduct interviews to gather relevant information and assess clients’ immediate needs (e.g., food, clothing, medical care, safety concerns).
- Complete comprehensive assessments to identify clients' strengths, challenges, and service needs (e.g., mental health, substance abuse, legal issues, financial stability).
- Case Management:
- Develop, implement, and monitor individualized service plans (ISPs) based on the client’s needs and goals.
- Coordinate and facilitate access to housing programs, employment services, health care, mental health support, and substance abuse treatment.
- Advocate on behalf of clients to secure necessary resources and benefits (e.g., public assistance, healthcare, legal services).
- Crisis Intervention:
- Respond to emergencies and provide crisis intervention as needed, offering support and de-escalation strategies.
- Ensure that clients’ safety is prioritized within the shelter environment.
- Documentation and Reporting:
- Maintain accurate and up-to-date case files, progress notes, and records of client interactions.
- Prepare reports on client progress, services provided, and outcomes as required by agency protocols or funding sources.
- Collaboration:
- Collaborate with other shelter staff, community organizations, and local agencies to ensure clients have access to a wide range of support services.
- Participate in multidisciplinary team meetings to discuss client cases and share resources.
- Follow-up and Support:
- Provide ongoing case management services to clients as they transition into stable housing.
- Monitor clients' progress and adjust service plans as needed to support successful transitions to permanent housing.
- Client Advocacy and Empowerment:
- Empower clients to take an active role in their recovery and housing stabilization by providing education, resources, and emotional support.
- Assist clients in building life skills (e.g., budgeting, job readiness, conflict resolution) to enhance their long-term stability.
Qualifications:
- Bachelor’s degree in social work, Psychology, Sociology, or a related field (or equivalent experience).
- Previous experience in case management, social services, or working with vulnerable populations, preferably in homelessness or housing services.
- Knowledge of community resources, social services systems, and crisis intervention strategies.
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team in a fast-paced environment.
- Ability to maintain confidentiality and adhere to ethical guidelines.
- Flexibility to work varied shifts, including nights, weekends, and holidays, as the shelter operates 24 hours a day.
Preferred Qualifications:
- Certification or training in trauma-informed care or similar methodologies.
- Experience with housing-first models, rapid re-housing, or other homeless services.
Physical Requirements:
- Ability to lift or move objects up to 25 lbs.
- Ability to remain standing, walking, or sitting for extended periods of time.
- Ability to respond to emergency situations in a timely and efficient manner.
Work Environment:
- The role is based in a 24-hour homeless shelter, which requires flexibility to work in a fast-paced and sometimes unpredictable environment.
- Must be able to work with individuals from diverse backgrounds and handle sensitive situations with empathy and professionalism.

Pay: From $17.00 per hour
