Interim HealthCare is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Case Manager. As a key member of our healthcare team, you will be responsible for providing personalized patient care to individuals in the comfort of their own homes.
You will work closely with a multidisciplinary team of healthcare professionals, including physicians, therapists, and social workers, to develop and implement individualized care plans that meet the unique needs of each patient.
The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. Additionally, you will need to have:
- A valid active unrestricted license as a Registered Nurse (RN) in the State of Michigan
- At least one year of clinical experience as a Registered Nurse
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
