Job Responsibilities:

• Deliver high-quality patient care in the comfort of their own homes
• Develop and implement individualized care plans
• Document patient progress and communicate effectively with healthcare teams
• Coach patients and families on disease management, medication adherence, and home safety
• Ensure seamless transitions and meet patient goals

Requirements for Success:

• Active, unrestricted RN license in the State of Michigan
• At least one year of clinical experience as a Registered Nurse
• Current CPR Certification
• Reliable transportation with a valid state driver's license and current car insurance
