We are seeking an experienced Registered Nurse (RN) to join our team as a Home Health RN Case Manager. In this role, you will provide personalized care to patients in the comfort of their own homes.

Responsibilities:
- Provide home-based care to patients with illnesses, injuries, and chronic diseases
- Develop and implement individualized plans of care for patients
- Work collaboratively with physicians and other healthcare professionals to coordinate patient care
- Educate patients and their families on healthy habits and disease management
- Conduct regular assessments and evaluations to ensure optimal patient outcomes
Requirements:
- Active, unrestricted RN license in Michigan
- At least one year of clinical experience as a Registered Nurse
- CPR certification
- Reliable transportation with a valid state driver's license and current car insurance
