Responsibilities
- Complete ongoing assessments of the patient's physical and psycho-social status
- Develop a plan of care in collaboration with the Interdisciplinary Group (IDG), patient/family, and attending physician
- Provide education and interventions to support effective pain management and other symptoms
- Ensure equipment, supplies, drugs, and biologicals are available
- Make revisions to the IDG plan as the patient's/family's status and needs change
- Document patient/family services accurately and timely
- Regularly attend IDG meetings
Requirements
- Active, unrestricted license in the State of Michigan
- At least one year of relevant experience as an RN or LPN
- One year of management experience (preferred)
- One year of home health experience (preferred)
- Current CPR Certification
- Community health and Medical-Surgical experience (preferred)
- Reliable transportation with a valid driver's license and current car insurance
