We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Hospice division in West Michigan.

Responsibilities
- Complete ongoing assessments of patients' physical and psycho-social status, and develop individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients/families, and attending physicians.
- Provide education and interventions to support effective pain management and symptom control, and assess patients'/families' responses to interventions and treatment.
- Ensure equipment, supplies, medications, and other necessary items and services are available, and make revisions to IDG plans of care as patients'/families' status and needs change.
- Document patients'/families' services provided accurately and timely, and attend IDG meetings regularly.
Requirements
- RN/LPN with active, unrestricted license in the State of Michigan.
- At least one year of relevant experience as an RN or LPN, with preferred experience in home health, community health, and medical-surgical settings.
- Current CPR certification, reliable transportation, and valid state driver's license required.
