In this role, you will work closely with an RN, LPN, CNA, HHA, OT, and SLP to provide home-based physical therapy services. Your responsibilities will include:
- Patient Assessment: Observe movements, conduct assessments, document progress, and modify care plans as needed.
- Hands-on Therapy: Assist patients with exercises and therapy to manage pain and increase mobility.
: Educate patients on their plan of care, established goals, exercises, and self-care.
To succeed in this role, you will need to possess the following skills and qualifications:
- Graduate of an Accredited Program: You must be a graduate of an accredited Physical Therapist program and hold an active PT license in Colorado.
- Minimum 1 Year Experience: A minimum of one year of physical therapy experience, ideally in home healthcare, is required.
- BLS/CPR Certification: BLS/CPR certification is mandatory, and a demonstration course will be required.
