Job Overview
We are seeking a dedicated Director of Case Management to oversee and lead our case management team in a Level II Trauma Center. The ideal candidate will have a strong background in managed care, emergency medicine, and possess knowledge in anatomy.
Responsibilities
- Manage and supervise the case management team to ensure efficient and effective patient care
- Develop and implement care plans in collaboration with healthcare providers
- Oversee discharge planning processes to facilitate smooth transitions for patients
- Utilize expertise in addiction counseling and mental health counseling to support patient needs
- Ensure compliance with regulatory requirements and standards in case management practices
Requirements
- Previous experience in a supervisory role within a healthcare setting
- Proficiency in managing care plans and coordinating patient care services
- Strong knowledge of anatomy, addiction counseling, mental health counseling, and discharge planning
- Familiarity with working in a Level II Trauma Center is preferred

Pay: $134,320.00 - $138,488.00 per year
