General Description
The Community Liaison serves as the primary point of contact between Journey and external healthcare providers, including hospitals, LTACHs (Long-Term Acute Care Hospitals), and other healthcare facilities. This role is responsible for building strong relationships, facilitating smooth transitions of care, and managing the admissions process for residents. The Community Liaison ensures that all admissions meet Journey’s clinical standards and regulatory requirements while also supporting the company’s growth by increasing referrals and occupancy rates.

Essential Duties
A. Relationship Development and Management:
- Builds and maintains professional relationships with hospital discharge planners, case managers, social workers, and other healthcare providers to generate referrals for Journey’s facilities.
- Represents Journey in a professional manner, promoting services, clinical expertise, and the benefits of long-term care.
- Conducts presentations to hospital teams and healthcare professionals to increase awareness of Journey’s capabilities and foster partnerships.
B. Admission Process Management:
- Manages the admissions process, coordinating with hospital and LTACH teams to ensure timely and smooth transitions of care to Journey facilities.
- Conducts clinical assessments of prospective residents, reviewing their medical records to determine appropriate placement within Journey's facilities.
- Collaborates with facility leadership and clinical teams to confirm that prospective admissions align with Journey’s clinical capabilities and regulatory requirements.
- Communicates with families and potential residents, providing education about Journey’s services and helping to facilitate the decision-making process.
C. Regulatory Compliance and Documentation:
- Ensures that all admissions comply with federal, state, and Journey’s internal regulations, including those related to Medicare and Medicaid.
- Reviews and verifies that all required admission documentation is complete, accurate, and in compliance with regulatory standards.
- Maintains thorough records of referral sources, admissions activities, and clinical assessments in Journey’s CRM or electronic medical records system.
D. Cross-Functional Collaboration:
- Works closely with Journey’s internal admissions, clinical, and business development teams to meet occupancy goals and ensure high-quality resident care.
- Provides regular feedback to clinical and administrative teams regarding referral trends, clinical needs, and potential areas for process improvement.
- Participates in regular interdisciplinary team meetings to discuss potential admissions, bed availability, and resident care needs.
E. Performance Metrics and Reporting:
- Tracks referral and admission statistics, identifying key trends and opportunities to improve Journey’s admission process and outcomes.
- Prepares and submits weekly and monthly reports on referral activities, conversion rates, and census growth to the Regional Vice President of Operations.
- Evaluates success in building partnerships and driving admissions, making recommendations for adjustments as needed.
Qualifications:
Education: Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred, or healthcare-related degree with clinical experience in long-term care or acute care settings.
Experience: 2-3 years of experience in healthcare sales, clinical liaison, admissions, or case management roles, ideally in a hospital, LTACH, or long-term care environment.
Skills: Strong knowledge of long-term care, post-acute care, and Medicare/Medicaid regulations. Excellent communication, assessment, and organizational skills.
Other: Ability to travel regularly to hospitals, LTACHs, and Journey facilities. Strong understanding of patient admissions processes and regulations.
