Job Description :
- Coordinate clinical and psycho-social activities with the interdisciplinary team and physicians to provide comprehensive care coordination and discharge planning for patients.
- Utilize critical thinking for effective discharge planning.
- Promote efficient utilization of clinical resources.
- Monitor all areas of patient stay for effective care coordination.
- Conduct medical necessity review for service utilization from admission through discharge.
- Document patient review records according to regulatory and legal requirements.
- Perform reviews for third-party payors as appropriate.
- Stay updated on reimbursement modalities, community resources, and relevant legal issues.
- Conduct biopsychosocial assessments of patients and families for discharge planning.
- Refer cases needing additional resource support to Social Workers.
- Serve as a patient advocate and collaborate to maximize patient and family decision-making ability.
- Participate in interdisciplinary patient care rounds / conferences.
- Collaborate with clinical staff on care plans and goal achievement.
- Demonstrate knowledge of lifespan growth and development principles.
- Coordinate social services to enable families to manage illness impacts effectively.
- Serve on division committees as requested.
- Initiate patient and family education and discharge planning at admission.
- Arrange discharge and post-hospital care within community institutions and agencies.
