Complex Needs Nurse Delegated Coordinator - Full Time Weekday

Summary

The primary role of the Nurse Delegated Coordinator is to provide support to clients on their caseload requiring nurse delegated and other medically intensive tasks. Assist clients to achieve their goals in accordance with the residential guidelines.

KIM is looking for qualified candidate to fill the following full-time weekday shift:

Shift One: Monday-Friday (8:00am-4:00pm) Total Hours: 40

Salary

$20.00- $21.00

Essential Duties and Responsibilities

• Provide Trauma-Informed, Crisis Intervention and Prevention Intervention Support.
• Support clients in achieving and identifying behavioral goals.
• Document client behaviors appropriately.
• Facilitate coordination of managed care.
• Acknowledge and advocate for the needs, wants, and rights of clients with dignity and respect.
• Identify areas of change that clients on their caseload would like to achieve.
• Coordinate individualized programs for clients and document activity thoroughly.
• Accompany clients on outings and activities.
• Transport clients to and from all agency activities, and/or arrange transportation services as appropriate.
• Assist with client money management such as petty cash, receipts, checkbook and EBT’s.
• Supervise clients to ensure adequate safety.
• Keep all hazardous materials, medications, etc. in the designated secured location.
• Implement a safe and clean environment in which our clients live by doing households ADLS (i.e., chores, meals, bathing, hygiene, removal of garbage, laundry, etc.).
• Report promptly instances of injuries, behaviors, abuse, and safety hazards, etc. to management, following the Chain of Command.
• Ensure adequate health care for each client.
• Assist clients with mobility needs, transferring them from wheelchair to bed.
• Monitoring vitals (if applicable) and client behavior.
• Turning or adjusting clients in bed to prevent bedsores and/or other discomfort.
• Ensure clients have access to regular meals.
• Accompany clients to health appointments and provide appropriate documentation for each appointment to management in a timely manner.
• Communicate with medical offices, pharmacies, and the nurse delegator.
• Administer medications to clients and document accordingly, including nurse delegated tasks.
• Assist client with daily hygiene as outlined in care plans, including personal care such as brushing teeth, bathing, hair washing, toileting, etc.

Certificates, Licenses and Registrations

• Certified Nursing Assistant (preferred but not required)
• Nursing Assistant Registered (required to obtain within 120 days of hire)
• Nurse Delegation Core Training (paid and provided by KIM)
• Home Care Aide (HCA) License (preferred but not required)
• Completion of the following trainings before working independently:
  • Trauma-informed practices
  • Crisis Intervention
  • Client specific on the job training
• Completion of the following trainings within 90 days of employment:
  • DSHS client specific needs training on a yearly basis
  • Dual Diagnosis Core Competency
• Relias Trauma Informed Care (paid and provided by KIM)

Education and Experience

• High school graduate or equivalent (required)
• Intermediate experience with supporting living, preferably with individuals developmentally or otherwise disabled adults.

Skills and Qualifications

• Minimum age of 18 years.
• Criminal background check cleared before working alone unsupervised with the clients.
• Valid driver’s license or State ID.
• Ability to follow instructions, organize and set priorities.
• Outstanding communication skills, including active listening, writing, speaking and reading comprehension.
• Being able to engage in critical thinking.
• Daytime shifts will require a reliable vehicle and car insurance OR a transportation plan.
