The Director of Social Services will lead the social services department in our long-term care facility, ensuring residents receive the necessary emotional, social, and psychological support to enhance their well-being. This role requires a skilled leader with experience in geriatric care and knowledge of long-term care regulations to manage a team of social workers and collaborate with other healthcare professionals to deliver comprehensive care to our residents.
Key Responsibilities:
- Oversee and manage the social services department to ensure residents' social, emotional, and psychological needs are met.
- Lead, supervise, and provide training and guidance to social workers, case managers, and other team members within the social services department.
- Develop individualized care plans in collaboration with residents, families, and interdisciplinary teams.
- Conduct assessments to determine residents' needs, including psychosocial, emotional, and environmental factors that may affect their well-being.
- Advocate for residents’ rights and ensure they have access to appropriate services and community resources.
- Maintain compliance with all state and federal regulations related to long-term care and social services.
- Work closely with family members to provide support and ensure clear communication regarding care plans, concerns, and updates.
- Facilitate resident and family meetings to discuss care needs, goals, and progress.
- Manage discharge planning for residents returning home or transitioning to other care settings, ensuring a smooth transition and appropriate follow-up.
- Develop and implement programs to enhance the quality of life for residents, including group activities, counseling services, and social engagement initiatives.
- Ensure proper documentation of all social services activities and maintain accurate records in compliance with facility policies and regulations.
- Manage the department’s budget and resources effectively, including grant applications and other funding opportunities.
- Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology.
- Current Social Worker license (LCSW, or equivalent) in VA (preferred).
- Minimum of [2] years of experience in social services, with at least [2] years of experience in a long-term care or geriatric care setting.
- Knowledge of local, state, and federal regulations regarding long-term care, social services, and healthcare compliance.
- Excellent leadership, organizational, and interpersonal skills, with the ability to work well in a team-based environment.
- Ability to develop and implement care plans and programs that meet the unique needs of long-term care residents.
- Strong communication skills, including the ability to work effectively with families, staff, and external agencies.
- Compassionate, patient, and client-centered approach to service delivery.
- Strong problem-solving and crisis management abilities.
- Proficiency in electronic health records (EHR) and Microsoft Office Suite.
