Qualifications:
- State of VA Licensed Registered Nurse or Licensed Practical Nurse.
- Must be able to attain CPR and AED certification within 90 days of hire.
- Ability to provide nursing services to the elderly which meets or exceeds Nursing Center’s quality care and service standards.
- Ability to effectively supervise a diverse employee work group when applicable.
- Ability to effectively communicate with residents, families, supervisor, and employees.
- Desire to work with the elderly.
- Ability to lift, move and transfer residents while providing nursing care.
- Knowledge/experience in long-term gerontological nursing care.
Key Duties & Responsibilities:
- Support the Mission, Values and Vision of LifeSpire of Virginia.
- Deliver direct nursing care in the clinic setting in accordance to state and federal laws/regulations and through application of acceptable nursing techniques, standards, objectives, and goals.
- Operates the clinic in accordance with the established goals and expectations and ensures the best care is available to residents utilizing clinic services.
- Ensures operation of the clinic during prescribed and published days and hours of operation.
- Assists in coordinating supportive services to include therapy, pharmacy services, and transportation. Collaborates and assists with the Care Coordination Nurse as needed.
- Provides in-home evaluation to a resident for sick call when the resident is unable to visit the clinic.
- Ensure compliance with regulatory agencies.
- Completes or assigns all treatments as prescribed by the attending physician.
- Performs services outlined in the most recent Virginia Nurse Practice Act and within scope of license.
- Prepares laboratory specimens with proper lab slips and identification and arranges for transportation to the appropriate place as soon as possible after collection. Obtain lab results after specimens are submitted for processing; notification of physician regarding results followed by proper documentation.
- Works with residents for ongoing or routine monitoring including medication review. Keeps these records up to date.
- Ensures all medical records are completed in a timely and accurate fashion after each visit.
- Establishes and maintains quality communications and a professional relationship with other care providers to include Nurse Practitioner, Physicians, Medical Director, etc.
- Adheres to all professional standards set forth in Lakewood’s Policies and Procedures.
- Participates in on-the-job training and regular in-service training sessions, including necessary fire drills.
- Always strives for quality improvement to ensure quality excellence, reduce risk liability and to enhance resident care and satisfaction.
- Maintains, aware of, and follows all written and verbally communicated expectations, policies, and practices.
- Perform other duties as assigned.
