Monday thru Thursday and Every other weekend 8:30am-5pm
Compensation:

OVERVIEW: The Patient Navigator acts as a member of a multidisciplinary team to assess and
coordinate patients’ care among multiple service providers during inpatient and outpatient
treatment.

- High School Diploma or equivalent.
- 1-year prior experience working with Substance Abuse populations strongly preferred
- Previous Case Management experience strongly preferred
- Valid, unrestricted driver’s license
- Clean driving record for at least 3 years
- Committed to drug-free lifestyle and responsible alcohol use
- Passion or interest in substance abuse issues strongly preferred
- Valid, unrestricted driver’s license
- Competency with Microsoft Office Suite.
- Comfortable working with electronic health records.
