Job Details
The Case Manager will maintain client chart records and documentation uploaded into the electronic health system. Track and ensure medical requirements for clients are obtained in the allotted timeframes. Communicate with prescribers as necessary and schedule monthly medication monitoring and work with outside providers. Worker with providers on trainings, medical appointments, and general support. Participate in treatment team meetings, admissions, and discharges. Support clinical staff with rotating on-call schedule. Hours
Position is full time and includes the following:
- 3.2 weeks of Paid Time Off during the first year
- Medical, vision, dental and life insurance

Education/Experience:
High School graduate with at least 3 years of experience.
