What are the responsibilities and job description for the Wellness Director, Assisted Living position at Liberty Park Senior Living?
POSITION SUMMARY: Performs resident health assessments; provides training/oversight to staff for medication assistance, health monitoring and assistance with health-related tasks, as allowed by regulation; coordinates resident care with third-party providers such as physicians and home health agencies. This position requires and understanding and commitment to the principals and values of the community. This position requires creativity and flexibility with residents who have complex situations and is at times stressful.
REPORTS TO: Executive Director
REPONSIBILITIES:
1. Adheres to principles of assisted living and conveys the philosophy of supporting the dignity, privacy, independence, choice, and individuality of residents.
2. Ensures the Community operates in accordance with all applicable regulations and community specific policies & procedures.
3. Performs assigned marketing and public relation activities to ensure a favorable public image of the Community is maintained and occupancy projections are reached.
4. Provides training and oversight to staff in the provision of medication assistance, health monitoring, and assistance with health-related tasks as allowed by regulation.
5. Correctly administers medications, documenting in the MAR. Assures that all medications are administered 100% of the time.
6. Provides training and oversight to ensure appropriate infection control procedures are followed by staff.
7. Conducts regular quality assurance reviews of staff documentation of medication/treatment assistance provided.
8. Coordinates and/or conducts move-ins and resident health assessments with new residents. Assessment schedule is upon move-in, 30 days after, and quarterly thereafter. Some residents will need to be re-assessed due to changes in condition.
9. Oversees or provides, in conjunction with the Executive Director, coordination with resident physicians and other health care providers
10. Oversees and provides coordination, in conjunction with the Executive Director, with the House pharmacy and outside pharmacies used by residents. Works with the Pharmacist to ensure accuracy of documentation of administrative records, physician orders, and proper labeling of medications.
11. Coordinates and oversees the implementation of all new physician orders.
12. Monitors the status of resident health conditions and initiates actions in response to changing needs; conducts re-assessments of resident health needs as indicated. Ensures the resident families, physicians, and others as appropriate are kept informed of changes in resident conditions and service needs.
13. Participates in the service planning and review process.
14. Assists the Executive Director with conducting and/or coordinating staff in-services and training, as requested. Assist the Assistant Director in the orientation of newly hired staff.
15. Investigates and responds to resident or staff concerns and reports them to the Executive Director and/or others as appropriate.
16. Works in conjunction with Executive Director to assure adequate staffing, supervising the implementation of plans of care, documentation, and management of specific resident problems.
17. Provides health counseling/education to residents as needed.
18. Performs TB tests for residents and staff.
19. Reports any abuse, suspected abuse, or alleged abuse to the Executive Director.
20. Is available and willing to be on an on-call basis to provide consultation to staff as needed and also to respond to staff concerns and questions. Arranges for appropriate coverage when unavailable.
21. Completes Incident Reports regarding accidents, unsafe or unsanitary conditions.
22. Follows hand washing policy and universal precautions.
23. Maintains compliance with state regulations for Assisted Living facilities.
24. Performs all other duties as assigned.
QUALIFICATIONS:
1. Desire to work with the elderly in an environment that upholds their privacy, dignity, choice, independence and individuality.
2. Good communication skills and the ability to relate to the public, residents, families, staff and other professionals appropriately and as outlined in the Employee Handbook.
3. Working knowledge of those principles necessary to provide competent nursing care.
4. Understanding of the normal dosages, actions and common side effects of most medications in common use.
5. Demonstrate leadership abilities and effective supervisory skills with the ability to direct the work of others. Good oral and written communication skills and the ability work well with others. Applies skills of problem solving techniques to promote cooperation and support from others.
6. Effective training and supervisory skills. Teaches awareness of fire and safety hazards, as well as health and sanitation hazards to staff, encouraging regular reporting of needed corrections.
7. The ability to identify, address and communicate problems effectively.
8. Working knowledge of and ability to conform to applicable laws, rules and regulations and community specific policies and procedures.
9. Professional appearance and demeanor.
10. General staffing requirements:

§ Able to physically/mentally perform tasks, as verified by a health screening that includes TB clearance
§ Criminal record clearance
§ CPR certification
§ Ability to read, write and understand English
§ General understanding of the needs of the elderly
§ Willingness to learn
EDUCATION AND EXPERIENCE:
Must be at least 21 years of age. Must have a current RN or LVN license for the State of Texas and be a graduate from an accredited school of professional nursing. At least one year experience working with the elderly or in a home-health setting required.
