What are the responsibilities and job description for the Case Manager position at Liberty Wellness?
The position is a full-time case manager position at our co-occurring outpatient treatment center Monday through Friday. The purpose of the position is to meet with numerous clients in the program to provide case management services.
Full-Time Case Manager Responsibilities and Duties
- Accurately assess client case management needs and prioritize case management assistance.
- Provide services such as helping clients access food stamps, cash assistance, disability services, rental assistance, etc.
- Assist clients with finding medical referrals and aid them in setting up their medical appointments.
- Aid clients with following through with their legal obligations including court dates, probation officer contact, legal treatment letters, etc.
- Assist with coordinating with clients for them to utilize and access medical needs while in the program.
- Aid clients with setting up aftercare plans.
- Provide necessary assessments to clients as needed.
- Produce all required documentation of case management services in an accurate and timely manner.
- Complete all trainings required for this position and attend any additional training identified by program management as necessary for the job.
- Ensure compliance with agency personnel file.
- All other duties as assigned.
Job Type: Full-time
Pay: $40,000.00 - $50,000.00 per year
