What You’ll Do:

- Work one-on-one with individuals facing challenges like appointment non-compliance, helping identify and solve underlying barriers.
- Conduct screenings and assessments in the community to connect individuals to the right care.
Collaborate and Innovate:
- Facilitate community collaboratives and share outcomes that inform organizational strategies.
- Educate providers and partners on effective ways to access mental health services.
- Support internal teams to meet deadlines and compliance requirements.
