What You’ll Do:

Support Individuals:

• Work one-on-one with individuals facing challenges like appointment non-compliance, helping identify and solve underlying barriers.
• Conduct screenings and assessments in the community to connect individuals to the right care.

Collaborate and Innovate:

• Facilitate community collaboratives and share outcomes that inform organizational strategies.
• Educate providers and partners on effective ways to access mental health services.
• Support internal teams to meet deadlines and compliance requirements.

Drive Impact:

• Provide crisis interventions, safety planning, and specialty screenings as needed.
