Job Description
Job Title : Licensed Practical Nurse (LPN)
Job Specialty : Long-Term Care (LTC)
Job Duration : 14 weeks
Shift : Full-time day / evening rotation, 5x8 per week. Every other weekend and holiday required.
Guaranteed Hours :
Experience : Minimum 1 year of experience as an LPN in a long-term care facility.
License : NY LPN Licensure
Certifications : Basic Life Support (BLS)
Must-Have : Demonstrated knowledge and skills to provide care appropriate to the age of the patients served on the assigned unit.
Job Description :
- Provide direct and indirect skilled nursing care for assigned residents.
- Follow defined policies and procedures under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with nursing staff, physicians, and ancillary staff.
- Maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
- Possess the ability to provide care to geriatric residents.
- Collect data relevant to residents' physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
- Contribute to the creation of a developmentally appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired outcomes, involving the resident / family and healthcare team while promoting a healthy, safe environment.
- Recommend revisions to the care plan after evaluation of resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
- Collaborate with the resident / family and members of the Interdisciplinary Team within the scope of nursing practice, assuring customer satisfaction.
- Maintain current knowledge and competency in Nursing Practice. Participate in the mentoring / orientation of staff.
- Consider factors related to safety, cost, and impact in planning and delivery of care, providing leadership in the professional practice setting under the direction of the RN.
- Occupational exposure to bloodborne pathogens is reasonably anticipated, with required tasks involving potential contact with blood, body fluids, or tissues.
