Job Description

Job Title : LPN Full-Time

Job Specialty : Long-Term Care LPN

Job Duration : 13 weeks

Shift : 5x8.0

Guaranteed Hours : 40 hours per week, every other weekend and holidays required

Experience : 1 year experience as LPN

License : LPN Licensure

Certifications : BLS

Must-Have :

  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served
  • Ability to provide care to geriatric residents

Job Description :

  • Provide direct and indirect skilled nursing care for assigned residents
  • Follow defined policies and procedures under the supervision of a Registered Nurse
  • Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff
  • Maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs
  • Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team
  • Contribute to the creation of a developmentally, age-appropriate, and culturally relevant plan of resident care reflecting nursing diagnosis and interventions
  • Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed
  • Collaborate with the resident / family and members of the Interdisciplinary Team within the scope of nursing practice, assuring customer satisfaction
  • Maintain current knowledge and competency in Nursing Practice; participate in the mentoring / orientation of staff
  • Consider factors related to safety, cost, and impact when planning the delivery of care
  • Under the direction of the RN, provide leadership in the professional practice setting
  • Demonstrate knowledge and competency concerning occupational exposure to bloodborne pathogens; tasks may involve potential contact with blood, body fluids, or tissues
