Job Title : Living Center LPN

Shift : Part Time, 3x8.0 shifts including every other weekend (Friday & Saturday nights)
Guaranteed Hours : 24 hours per week
Experience : 1 year as an LPN in a Long Term Care Facility
License : NY LPN Licensure
Certifications : BLS
Must-Have : Experience providing care to adult and geriatric residents
Job Description :
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
- Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care, reflecting nursing diagnosis and interventions to achieve desired outcomes.
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of care are addressed.
- Collaborate with residents, families, and members of the Interdisciplinary Team within the scope of nursing practice to ensure customer satisfaction.
- Maintain current knowledge and competency in Nursing Practice through continuing education and in-service programs.
- Participate in the mentoring and orientation of staff and provide leadership in the professional practice setting under the direction of the RN.
- Consider factors related to safety, cost, and impact in planning and delivery of care, demonstrating knowledge and competency.
- Anticipate occupational exposure to blood borne pathogens and handle tasks involving potential contact with blood, body fluids, or tissues with appropriate precautions.
