The Young Parent Case Manager works under the direction of the Young Parent Programs Coordinator. This position will successfully engage and retain young people between 16 and 25 in services. They will identify the strengths and needs of young people who are parenting and/or pregnant. The Young Parent Case Manager connects young parents to community resources to meet their needs. This position will gather information from young people via screenings, assessments, and consultation with collateral sources. With the young parent, the Case Manager will develop goals for an individualized service plan. Evaluation of progress toward these goals as well as discharge planning and implementation are expected. The Young Parent Case Manager will ensure timely assessment of child(ren)’s development. This position will build connections between LUK and key partners and work with young people to achieve their goals while adhering to LUK’s core values.
A Bachelor’s Degree in Human Services or related field is preferred, but relevant experience may be substituted. Experience with case management, and/or experience working with young parents preferred. Strong relationship-building skills required. Excellent organization, communication, and computer skills required. Valid driver’s license and own transportation required.
Full-time positions located in Fitchburg and Worcester are available. Positions require community-based travel and implementation.
