Program Description:
Folsom Dore Apartments is a 98-unit permanent housing program for adults located in the South of Market Area (SOMA) of San Francisco, with 40 units dedicated to formerly homeless individuals. The program serves primarily low-income individual with histories and challenges around chronic poverty, unemployment, access to education, immigration status, and domestic violence. The services team is comprised of one full time Program Coordinator and one full time Case Manager, supervised by the Adult/Senior Housing Program Manager. The team works together to ensure all households are provided with necessary resources to ensure individual resilience and housing stability.

Essential Duties and Responsibilities:
- *Provide Case Management services and Coordinate all regular program activities for the Folsom Dore Apartments Housing Program.
Core functions include the following:
1. -
· Provide case management services, including counseling, crisis intervention, therapeutic support, psycho/social education, addressing lease violations, conflict and behavioral management, resources, and other assistance as needed for individuals with a history of chronic homelessness.
· Participate in case conferences with other service providers as needed.
· Provide assessments, case plans, and follow up in collaboration with residents based on their individual need and goals.
· Provide individualized budgeting, education on problem-solving techniques, communication skills and home management to residents.
· Maintain case records of client caseload and other record keeping as required by the agency.

· Work with on-site TNDC staff to plan, create, and implement building wide community events focused on individual enrichment.
· Outreach to residents in the form of event fliers, monthly calendars, weekly schedules, and conduct door to door outreach to encourage community participation.
· Work with community partners to facilitate bringing in outside resources in the form of workshops, training, and other events to create rich and diverse programming, events and activities that support life, education, and employment goals.
· Coordinate and partner with SF Marin Food Bank to conduct the weekly food pantry. Take lead on the distribution of the coordination and facilitation of this event.

· Take the lead in fulfilling reporting requirements for the program site in the form of annual monitoring, monthly deliverable reports, outcome measurements and other reporting required by funding sources and agency needs – including navigating the agency’s Case Management tool and San Francisco Coordinated Entry database.
· Serve as the on-site support services liaison to Property Management. Coordinate agendas and co-facilitate weekly meetings with property management (TNDC). Coordinate and support monthly resident community meetings.
· Conduct an annual satisfaction survey and implement resident feedback into program ideas as appropriate.
· Create and coordinate agenda and facilitate weekly team meetings with the onsite Case Manager and the Adult/Senior Housing Program Manager.
· Other duties as assigned by supervisor.
Qualifications:

· BA or BS degree and 4 years’ experience in social work, preferably in the non-profit or public sector; or equivalent combination of education and lived experience. Bachelor’s degree in Social Work or related field preferred.
· Experience in accessing community-based services and ability to relate professionally to other service providers. Knowledge of resources that support the resident population.
· Ability to work in a fast-paced environment that requires strong multi-tasking capabilities.
· Knowledge and experience in working with culturally diverse populations.
· Excellent writing and analytic skills.
· Demonstratable basic computer skills, MS Word, Excel, Outlook, Internet.
· Able to work independently, use common sense, and utilize supervision as needed.
· Ability to work collaboratively in a team setting.
· Experience with group facilitation and community building.
· Bilingual capacity a plus (Cantonese/Mandarin or Spanish)
