What are the responsibilities and job description for the Home Health Quality Assurance nurse / QA nurse position at Luxe Home Health?
Quality Assurance Nurse is responsible for review of clinical records for consistency, compliance with regulation, quality of documentation, coordination of the multidisciplinary plan of care and clinical outcomes.
This position works closely with the Director of Nursing and Agency Administrator to ensure that documentation is submitted in a timely manner, properly coded and meets a quality standard.
Key Responsibilities
Quality Improvement:
- Completes clinical record review at start of care, resumption of care, recertification, and discharge for complete and accurate documentation, development of appropriate discipline-specific care plan, accurate physician orders, and coordination of care and compliance with Medicare, ACHC, and state regulations.
- Completes all documentational reviews within agency-defined timeframe.
- Demonstrates understanding of current Medicare guidelines and ensures that clinical records are compliant with COPs; maintains agency in survey-ready status.
- Demonstrates proficiency in use of the EMR (AXXESS) and other web-based programs if needed.
- Provides expertise in OASIS, record's review, participates in staff education and onboarding programs.
- Works in direct collaboration with Director of Nursing and participates in Star rating improvement and QAPI programs.
- Maintains proficiency in OASIS dataset and data capture best practices, according to industry guidelines and CMS regulations;
Patient Care Coordination and supervision:
- Communicates with all disciplines regarding patient services to ensure continuity of care
- Participates in weekly Case conferences
- Performs Verification of medications if needed
- Manages Lab requisitions and follow up
- Providing Back-up to the Director of Nursing
