Job Position: Life Bridges Program Coordinator
Full- time: hourly 35 hours/week
REPORTS to: Life Bridges Program Manager
The Life Bridges Dementia Day Program at MAC focuses on quality of life for people with Alzheimer’s or other dementia, and their caregivers. Housed in a specially designed, secure wing of the building, Life Bridges offers a robust set of activities designed to enrich
the lives of those who attend.

Job Description
· Under the direction of the Life Bridge Program Director, the Program Coordinator will assess and manage facilitation of comprehensive activities and care program with other team members and volunteers
· Assists in planning, scheduling, and conducting programs that provide physical, intellectual, social, emotional and reminiscing opportunities for the members.
· Assists, invites, and encourages members to participate in activities.
· Helps plan appropriate programs for holidays and special events and coordinates holiday decorations for the center
· Coordinates with other Senior Center programs to ensure that all equipment and supplies are available for activities and special events
· Assists in maintaining an inventory of activity and programming supplies.
· Works with other Senior Center programs to prepare and organize calendar of events.
· Submits the calendar to the Program Director for final approval. Posts and distributes the calendar
· Maintains the daily member log, assessments, and participation. Provides feedback to caregivers as needed
· Greets new participants and families to introduce the program
· Oversees provided meals for participants.
· Follows all policies and procedures.
· Ensures that all members are always accounted for while in attendance.
· Participate in conferences and training opportunities as required
· Participate in staff team meetings

Educational Requirements and Experience
· Associate degree or equivalent from two-year college or related training
· One-three years preferred in assisted living, long term memory care or experience/exposure to the senior population.
· Care experience with individuals with Alzheimer’s or related dementia
Knowledge, Skills, and Abilities
· Ability to communicate effectively in writing and verbally
· Patience and sense of humor a must!
· Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
· Ability to write routine reports and correspondence.
· Ability to speak effectively before public groups or MAC employees
· Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
· Ability to use Microsoft Windows, including Word and Excel. Ability to use software to design calendars and signs.
· Ability to use email and the Internet.

Working Conditions: Works in and out of temperature-controlled environments, which may become warm, congested and busy at times. May be required to bend, sit, stand, walk, exercise for short/long periods of times and operate specific office equipment. Must regularly lift and /or move up to 10 pounds, and occasionally lift and/or move up to 25 pounds. Close vision and distance vision are need for this job.

Evaluation:
· Methods: Evaluation form with review by Program Director
· Frequency: 90 Day for a new employee and annually thereafter.

Pay: $16.00 - $18.00 per hour
