Description
With general directions, plans, organizes, delivers independent counseling, and monitors medical conditions of high risk prenatal clients at home and in clinic settings. Candidate will assure that all program standards and procedures as outlined by the Illinois Department of Public Health, Department of Human Services and East Side Health District are met. Candidate will provide case management services, nutrition education, conduct health education and implement appropriate action to minimize risk factors and promote a healthy pregnancy outcome. Maintaining confidentiality is an absolute must.
- Position is contingent upon continued funding of present grants.
Qualifications
- Registered Nurse-Diploma, Associate, Baccalaureate or Master’s Degree in Nursing. Must submit an Illinois R.N. License or apply for Illinois reciprocity;
- Master’s in Social Work;
- One year experience providing services to pregnant women & families with young children;
- Ability to maintain satisfactory working relationships with other employees, patients, family members, health personnel, and other professionals;
- Capable of working with individuals and groups to promote good health and disease prevention;
- Requires thorough knowledge, skills and experience working with and utilizing community resources;
- Ability to speak in Spanish will be given preference;
- Ability to conduct effective assessment interviews and develop care plans;
- Ability to develop and implement treatment procedures (individual care plans);
- Computer literacy at the user level required;
- Candidates must own a dependable automobile and possess a valid driver’s license and auto insurance;
- Will accept any other duties as assigned by ESHD;
