Job Summary

We are seeking experienced Nurse Practitioners to join our team as Comprehensive Health Assessment Nurses. As a Comprehensive Health Assessment Nurse, you will conduct home risk assessments, provide medical history and diagnosis, and develop treatment plans tailored to the individual's needs.

Key Responsibilities

• Conduct comprehensive health assessments, including medical history, diagnosis, and treatment planning.
• Collaborate with physicians, case managers, and other healthcare professionals to ensure comprehensive care.
• Develop and implement treatment plans that address the individual's unique needs and circumstances.
• Provide education and support to patients and their families to promote healthy behaviors and self-management.

Qualifications

• Masters Degree in Nursing or related field.
• Current RN and NP licensure in state of practice.
• Board certification by AANP, ANCC, or AACCN.
• 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with electronic health records.
