The Community Care Case Manager is responsible for engaging and assisting patients on their journey toward sustainable long-term improvements by breaking down social barriers to enhance whole-person health and wellness. This role involves advocacy, guidance, and support, collaborating closely with clients, healthcare teams, and the CalAIM Supervisor. You will partner with clients seeking self-directed, lasting changes that align with their values and care plans, particularly those facing substance use disorders, chronic conditions, and other health challenges. Through the Enhanced Care Management (ECM) program, you will facilitate better health outcomes and empower clients to actively participate in their recovery.

Requirements
MINIMUM QUALIFICATIONS:
- Experience or education in health promotion, social work, public health, or other related fields.
- Pursuing or holding a certification in addiction counseling, Peer Support Specialist, or Health Coach; OR experience in nursing, social work, or public policy.
- Knowledge of substance use disorders, chronic conditions, and lifestyle interventions.
- Valid California Driver’s License and auto liability insurance.
PREFERED QUALIFICATIONS:
- Knowledge of group process, psychosocial concepts, support systems, stages of change, Motivational Interviewing, ethical standards, and medical and behavioral health care systems
- Bilingual English/Spanish (fluent)
