Job Description
Responsibilities / Job Duties :
- Initiates outreach efforts to potential enrollees, providing information on Long-Term Services and Supports Care Plan options and the role of LTSS Care Coordinators.
- Uses motivational interviewing techniques to engage and inform potential enrollees about the program and enrollment process.
- Performs unscheduled home visits as needed, particularly if no contact is made within 90 days of the initial outreach.
- Tracks and manages outreach follow-up communications and documentation.
- Obtains participation forms from enrollees who agree to join the Long-Term Services and Supports Care Plan program to confirm their consent.
- Schedules intake appointments for enrollees with Intake staff.
- Maintains quality communication with enrollees, internal staff, and external partners (including ACOs and physicians).
- Actively participates in training activities, including shadowing Care Coordinators and working with a mentor during orientation.
- Documents all activities and outreach efforts in the electronic health record system (eHana).
- Prepares and distributes informational packages for home visits with enrollees.
- Regularly communicates with NRLP Program Coordinator and other NRLP staff about outreach status.
- Manages, tracks, and distributes outreach materials such as brochures.
Minimum Education and Experience Qualification Requirements :
Bachelor's degree in a related field.
