What are the responsibilities and job description for the Case Manager position at MedPlex MD Injury?
Case Managers perform several tasks to achieve the best outcomes for our patients. These duties usually involve working with our patients directly and often working with the patient’s attorney and health care workers.
Key Responsibilities: (may include, but not limited to):
- Listening to patients concerns and providing counseling or intervention as required
- Recording clients’ progress including charting referrals and other notable interactions
- Managing medical charts and records
- Evaluating clients’ progress periodically and adjusting as needed to improve outcomes
- Become the point of communication between a patient and their legal team in their legal matters regarding treatment and needs.
- Constantly updating/communicating with patients’ legal team to ensure proper liability and coverage is available for patients’ treatment
- Following up with discharged clients to ensure they were satisfied with services, and they are still in good physical and mental health
- Executing subpoena requests as needed
- Effectively manage multiple cases at any given moment ensuring the best care possible for each patient
