Full Time 35 Hours per week
Seeking motivated and organized Care Manager for MHA's new Family Centered Care Management program. This program aims to assist families, designated and referred by DCDCFS, who are homeless and placed in temporary housing by DCDCFS, currently applying for or in receipt of TANF or federally funded SN assistance. The ideal candidate will assist individuals and their children with complex needs around navigating state and federal benefits programs, facilitating ongoing program compliance, and providing targeted support for at-risk clients with a history of non-compliance.
Requirements
- Bachelor’s or higher from a regionally accredited or NYS registered college or university
- A background in provision of direct client services in their own homes
- Good communication and counseling skills with individuals and children
- Excellent time management skills
- Two (2) years of relevant work experience
- Valid NYS Driver's License
- Bilingual Spanish a plus
- Knowledge of community resources a plus
Job Type: Full-time
Pay: $26.92 per hour
