We are seeking a full-time Case Manager to join our Dual Diagnosis Treatment Team (DDTT) based out of West Lawn, PA.Earn $23.73 / hour, plus mileage reimbursementSchedule : 1st shift, Monday-Friday, plus on call rotationPosition DetailsThe case manager plays a pivotal role as the recovery coordinator for our community-based Dual Diagnosis Treatment Team (DDTT) serving Berks County. The DDTT focuses on a recovery-oriented approach supporting individuals diagnosed with both a mental illness and intellectual disability.Duties and ResponsibilitiesCollaborate with other members of the team to the implement recovery-oriented, person-centered treatment and support plans.Performing initial intakes, assessments, and timeline summaries.Provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
