Schedule: Monday-Friday, 8:30 AM - 5:00 PM, with occasional weekends, evenings, and crisis response as needed.
Job Summary: The Housing Solutions Case Manager (PSH) is responsible for conducting needs assessments, creating and implementing housing/emergency plans, providing support, and offering advocacy for individuals in the Mercy House Permanent Supportive Housing Program. This role requires building supportive relationships with residents to promote self-reliance and stability, utilizing trauma-informed, client-centered strategies.
Essential Duties and Responsibilities:
- Client/Service Delivery:
Manage a caseload of 17-25 residents, meeting bi-weekly to develop housing support and emergency plans. Apply harm reduction, trauma-informed care, and Motivational Interviewing techniques. Conduct health and safety inspections and make appropriate referrals to community resources. Facilitate social activities to encourage peer support. Participate in the on-call schedule and collaborate with community support providers.
- Team/Staff Relations:
Collaborate with Mercy House staff and volunteers to advance the agency’s mission, attend meetings, and contribute to program development. Adapt to a fast-paced environment and communicate effectively with a diverse population.
- Administration:
Maintain accurate documentation, including housing and emergency plans, case notes, and Housing Authority paperwork. Ensure timely submission of paperwork and assist with data collection. Meet deadlines and prioritize tasks efficiently.
Requirements:
Education & Experience:
- Bachelor's degree in Social Work or related field preferred. Candidates without a degree but with at least 2 years of relevant experience working with homeless or low-income populations are encouraged to apply.
- Understanding of confidentiality, case management guidelines, and ethical issues.
- Proficient in Microsoft Office and other relevant systems.
General:
- Strong communication, crisis intervention, and organizational skills.
- Ability to maintain professionalism in a diverse environment.
- Fluency in Spanish is highly valued.
Physical:
- Ability to lift up to 50 pounds and sit for extended periods.
- Reliable transportation is required.
