Job Summary: The Housing Solutions Case Manager is responsible for conducting ongoing needs assessments, creating and implementing personal treatment plans, providing support, offering information and referrals, and advocating for individuals in the Mercy House Permanent Supportive Housing Program. This position plays a critical role in fostering positive relationships with residents to help them increase their self-reliance. The Case Manager will identify challenges that may hinder residents' self-reliance and housing stability while reinforcing positive behavioral changes. Accurate record-keeping is a crucial part of this role. Additionally, the Case Manager will ensure the team applies trauma-informed, client-centered strategies throughout their work.
Essential Duties and Responsibilities:
Client/Service Delivery:
- Maintain a caseload of at least 15 individuals, meeting with each resident a minimum of twice a month to develop and monitor their stabilization plan.
- Implement harm reduction interventions to provide client-centered services and promote housing retention.
- Apply crisis prevention and intervention skills as necessary.
- Practice Trauma-Informed Care when interacting with clients.
- Support residents in recognizing and building upon their strengths while addressing their needs with respect.
- Conduct apartment visits and inspections to ensure proper maintenance.
- Stay informed about community resources and make appropriate referrals for clients.
- Organize and facilitate social and recreational activities to foster community involvement and peer support.
- Collaborate with volunteers and community resources to ensure residents are linked to services such as employment, education, tutoring, and/or mentoring.
- Attend relevant training to enhance knowledge in areas like co-occurring disorders, harm reduction, and cultural competence.
- Facilitate relationship-building between residents, landlords, and caretakers.
- Participate in the emergency on-call schedule to address client needs outside of regular hours.
Team/Staff Relations:
- Collaborate as a member of the Mercy House team to uphold the agency's mission and core values.
- Participate in regular internal and external agency meetings.
- Contribute to planning and developing resident programs as part of the team.
Administration:
- Maintain accurate and up-to-date documentation on clients, including stabilization plans, case notes, and other relevant records.
- Complete paperwork in a timely and precise manner to ensure proper agency payment from various sources.
- Assist with data collection systems as needed.
- Demonstrate a willingness to gain knowledge about Adult Rehabilitative Mental Health Services and Substance Abuse Treatments.
- Perform other duties as assigned.
Requirements:
Education and Experience:
- A Bachelors degree in Social Work (or a related field) with experience working in chemical health, mental health, or co-occurring disorders is preferred.
- Candidates with less than a BA/BS but with at least 2 years of direct experience working with long-term homeless, low-income, and diverse populations, and a working knowledge of mental health or related issues, are also encouraged to apply.
