$2,500 Signing Bonus: $500 within your first paycheck, $1,000 after completing six months of employment, and final payment of $1,000 after 1 year of employment.

We are now hiring a full-time Case Managers for our Midway Center!

ESSENTIAL JOB FUNCTIONS:  

  • Using intermediate level skills, conducts comprehensive client assessment: works with more complex cases.
  • Develops a written service plan with the client to meet client need, specifying goals to be achieved and services to be pursued.
  • Supplements/service plans to meet client needs.
  • As appropriate, provides advocacy, case management, and supportive counseling to individuals, families, and groups in coping with environment and interpersonal relationship issues.
  • Uses expanded knowledge of resources and helps clients utilize them. Works collaboratively with community resources.
  • Handles emergency and crisis situations within target population.
  • Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
  • Reassess and monitors client situation and adequacy of service plan.
  • Collaborates and participates with other program staff in the planning and implementation of service plans.
  • With supervisory approval, authorizes limited amounts of financial assistance to own clients.
  • Complete program registration and scheduling for behavioral health. This includes but is not limited to benefits verification, screening, and triaging of clients, and ensuring that the clients are scheduled for first appointments in a timely manner.
  • Provide resources and information to avoid escalation of symptoms or needs for individuals and families waiting to be assigned to a BH team.
  • Monitor the BH waitlist in collaboration with the BH Coordinator and assist with registration, enrollment, and scheduling of clients transitioning from the waitlist into services.
  • Support the delivery of psychiatry services by completing vitals of clients prior to their appointments, assisting clients with accessing telehealth services, and collaborating with other BH team members to provide medication training or medication monitoring services to clients who are receiving psychiatric care and are in need of resources, education, or other support related to medication management.
  • Supports the Behavioral Health teams by coordinating other services as assigned, such as group services or community support outings.
  • Coordinating group services.
  • Identifies systems issues and reports to Program Director as appropriate. May work with community groups and may carry a special community assignment.
  • Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
  • Maintains competencies in working with diverse populations in order to successfully engage clients in assessments, service/care planning and referral and linkage.
  • Completes documentation/data entry on services rendered, attempted contacts, staffing, supervision, etc. in EHR and health population platforms in a timely manner.
  • Works effectively with diverse staff and service populations.
  • Other duties as assigned.

QUALIFICATIONS:

  • Bachelor's Degree in Social Work or related field from an accredited college or university required. 
  • One year of case management experience in a social service or healthcare setting, required.  
  • Bilingual in Spanish required. 
  • Crisis stabilization experience preferred. 
  • Demonstrated ability to render adequate service to clients and to organize and manage job efficiently.
  • Must have access to a vehicle, valid driver's license and proof of insurance to perform essential job functions, required. 
  • Employment with MFS requires proof of vaccination or an approved Medical exemption, required.
  • Requires mobility to work with community-based caseload. 
  • Ability to maintain assigned community and home-based services. 
  • Ability to work effectively with diverse individuals and groups.
  • Ability to work evening and weekend hours as needed.

    PHYSICAL DEMANDS 

    • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.
    • The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
    • The employee may occasionally lift and/or move up to 50 pounds.

    SALARY: 

    • The average starting salary for this position will fall in the range of 40,000 to 50,000 annually. Where candidates fall in this range will be based on skill and experience level.
