We are now looking for skilled and compassionate QMHP-As to join our team in providing exceptional care for individuals receiving mental health and substance abuse treatment in our residential crisis stabilization community. What You’ll Do:
As a Care Coordinator (QMHP-A) on our team, you will work closely with other healthcare professionals to assist clients/patients in achieving stabilization and reintegration. Your primary responsibilities will include:
Duties and Responsibilities:
Collaboration with Healthcare Team: Work with administration, clinicians, nurses and and other professionals to coordinate individualized care plans and participate in treatment team meetings. Assess, plan, and monitor the client's status and needed community service coordination
Supportive Services: Provide support to the clients while attending sessions
Emergency Response: Respond to emergency situations and coordinate care with external emergency services when necessary.
Documentation: Maintain accurate and up-to-date client records in accordance with regulatory requirements. Process new referrals, verify client insurance, schedule assessments, and/or refer clients to community resources as needed
Care Coordination: Accurately manages and tracks new client referrals, meeting the goals established by the Program Director. Connect clients with clinical and social resources, supporting discharge planning and follow-up care.
Compliance: Ensure adherence to regulatory requirements and participate in quality assurance activities to improve care.
Quality Assurance: Ensure all services related to counseling, client care, treatment, service plans, etc., are of the highest quality and with a client-first mentality
Safety: Provide a safe and secure environment for crisis intervention.
Qualifications:
- Bachelors Degree from an accredited university in a human services field
- Current QMHP-A certification
- Current CPR/First Aid Certification
- Experience in behavioral health/residential care/addiction recovery preferred
- Strong interpersonal and communication skills
- Ability to work collaboratively in a fast-paced environment
- Commitment to trauma-informed, patient-centered care
