What are the responsibilities and job description for the QMHP position at The Empowerment Center?
The Qualified Mental Health Professional – Adult/Eligible (QMHP-A/E) will provide direct, community-based mental health skill-building services to adults with serious mental illness. This role focuses on enhancing daily living, social, and coping skills to help clients achieve and maintain independence in the community. The QMHP-A/E will work with clients individually, tailoring interventions to meet their needs and service goals outlined in treatment plans.
- Provide mental health skill-building services to adults in their homes and communities, focusing on activities of daily living, medication management, and social skills.
- Implement person-centered treatment plans, ensuring goals and interventions align with the client’s strengths and needs.
- Facilitate skill-building in areas such as independent living, problem-solving, self-care, and employment readiness.
- Monitor and document clients' progress, updating treatment plans as necessary in coordination with the clinical team.
- Support clients in crisis intervention and safety planning when needed, following agency protocols.
- Maintain accurate and timely records of all client interactions and progress notes according to Medicaid and agency standards.
- Submit required reports, assessments, and documentation by agency and regulatory deadlines.
- Collaborate with other providers, including case managers, medical professionals, and support staff, to ensure a holistic approach to client care.
- Participate in multidisciplinary team meetings, case reviews, and supervision sessions as scheduled.
- Communicate effectively with clinical supervisors, ensuring transparency regarding client challenges, progress, and any safety concerns.
- Establish positive working relationships with clients' families, community partners, and external support networks as needed.
- Act as an advocate for clients, assisting them in navigating community resources such as housing, employment, and healthcare.
- Empower clients to set and achieve personal goals, providing ongoing encouragement and support.
