Lead Clinical Coordinator
Behavioral Health Services – ACT Team
Hourly Range $30.00 - $44.37
Job Summary: The Lead Clinical Coordinator will provide clinical oversight, ensure program growth, and follow Community Support Program (CSP) statutory requirements under DHS 63 for admission, clinical supervision, crisis management, and ongoing case management. This position will conduct intake assessment and planning, crisis management/planning, treatment planning, coordination of care, clinical supervision of staff, and provide comprehensive, direct, clinical care to individuals with serious and persistent mental illness. The Lead Clinical Coordinator is necessary to increase the Assertive Community Treatment (ACT) program census and oversee clinical needs as well as risk mitigation.

Job Duties
- Conduct individual assessments, including in-depth assessment at time of intake. Complete routine assessments thereafter at least every six months. Provide oversight of staff assessments and planning, ensuring alignment with DHS 63 standards, and establish rehabilitative goals to further recovery.
- Provide direct case management services to help consumers successfully live in the community. Provide brokerage of services outside of ACT team, regular collateral contacts of providers, and others involved in the consumers' lives.
- Provide crisis response services to individuals who are experiencing a psychiatric crisis. Interface with county operated crisis services, hospitals, and others as needed. Includes being part of a rotation of after hours on call services provided by the ACT team.
- Provide individual and group therapy as well as psychoeducation groups to ACT team consumers by using Motivational Interviewing, Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, and other evidence based therapeutic practices. Enhance the consumers’ independent living skills.
- Complete clinically appropriate and timely documentation of all services rendered on behalf of consumers served by the ACT team in the Electronic Medical Record. Maintain a productivity level of 75% or greater.
- Support the Integrated Services Manager to lead daily team meeting, help resolve clinical problems, ensure quality of treatment provisions, and oversee staff accountability.
- Travel of up to 75% of time within the community is required.
- Requires being part of a rotation for after hours and weekend on-call services.
- Perform other duties as assigned to support the care and services of clients being served by the ACT team.
- Current resident of the state of Wisconsin at the time of application
- Valid Wisconsin driver’s license and access to a reliable insured vehicle, both maintained throughout employment
- Master’s Degree in Human Services, Psychology, Social Work, Occupational Therapy, or in Nursing (RN) is required.
- At least five (5) years of related experience providing services to individuals with Mental Health and Substance use disorders is required.
- At least intermediate experience with all related computer and software such as Microsoft Office Suite programs
- Available and willing to be part of a rotation for after hours and weekend on-call services as well as travel within the community up to 75% of work time.
- Licensure as a LPC, LCSW, LMFT or 1,500 hours of supervised clinical experience in a CSP is required.
- Clinical knowledge of the needs of individuals with serious and persistent mental illness, co-occurring disorders, and the civil legal system is required.
