Job Overview
We are seeking a compassionate and dedicated Hospice Nurse Practitioner to join our team. In this role, you will provide high-quality, patient-centered care to individuals facing life-limiting illnesses. You will work collaboratively with a multidisciplinary team to ensure that patients and their families receive the support and medical attention they need during this critical time. Responsibilities
- Conduct comprehensive physical examinations and assessments of patients in hospice care.
- Develop and implement individualized care plans based on patient needs and preferences.
- Perform specimen collection processing as required for diagnostic purposes.
- Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life care.
- Ensure compliance with HIPAA regulations to protect patient confidentiality.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate care.
- Manage pain and symptom relief using appropriate interventions, including DME (Durable Medical Equipment) as needed.
- Assist with family planning discussions as appropriate for patients' situations.
- Document all patient interactions and care provided in accordance with regulatory standards.
Qualifications
- Current Nurse Practitioner license in the state of practice.
- Strong understanding of assisted living environments and their impact on patient care.
- Excellent communication skills, both verbal and written, with an emphasis on empathy and compassion.
- Ability to work effectively within a multidisciplinary team while demonstrating leadership qualities.
