Job Title: Integrated Case Manager
Location: Detroit, MI
Duration: 13 weeks (Temp – Perm)
Shift: 8:30am - 5pm (5x8)
Gross Weekly: $1900 - $2200
Duties:
- Conduct comprehensive assessments of patients and families.
- Develop personalized patient-centered care plans.
- Engage patients and families through advocacy and education.
- Utilize critical thinking and motivational interviewing to address barriers.
- Provide counseling on treatment decisions and end-of-life issues.
- Coordinate transitions between care settings.
- Collaborate with the care team on complex cases.
- Continuously evaluate and modify care plans as needed.
- Participate in process improvement activities.
- Address cases involving abuse and neglect.
- Perform additional duties as assigned.
Required:
- Clinical Background.
- 3-5 Years Case Management Experience.
- Strong Computer Skills.
- Experience In Discharge Planning, Home Health Care, Rehabilitative Medicine, Community Health, Or Managed Care.
Certifications/Licenses:
- RN or LMSW License
- BLS-AHA
Salary : $1,900 - $2,200
