Your Role

As the Healthy Families Coordinated Intake Specialist at Near North Health, you’ll serve as the vital gateway for families accessing our life-changing Home Visitation Programs. Using your expertise and compassionate approach, you’ll screen families, identify their unique strengths and needs, and connect them to the right services. By building strong relationships with community partners and fostering seamless collaborations across departments, you’ll help promote healthy families and prevent adverse childhood outcomes. Your role will ensure families receive the care and resources they need, creating a lasting positive impact on our community.

Qualifications

  • Education Requirements: BS/BA in Human Services field.
  • Required Licensure and/or Certification: CPS12, Visit Tracker, and Cornerstone
  • Years of Experience: Two or more years of experience in Social/Human Services field. Program experience and some quality improvement and quality assurance experience desirable. Demonstrated knowledge of community resources and social services providers. Excellent verbal and written communication skills. Computer operations and data entry skills.

Compensation

Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role. Candidates who meet the minimum requirements will typically start at the entry level of the range, with additional qualifications reflected in the final compensation package.
