What you'll Do: 
  • The Bridges of Self-Sufficiency (BOSS) Self-Sufficiency Mentor is pivotal in transforming the lives of individuals and families navigating poverty in Northeast Michigan. This role is dedicated to providing comprehensive case management services and mentorship to participants of the BOSS program. By building trust and guiding clients towards self-reliance, the Case Manager/Mentor will oversee a caseload, offering tailored support, resources, and encouragement every step of the way. Responsibilities extend to active involvement in the community, requiring regular travel across the 11-county service area to conduct meetings, deliver presentations, and participate in professional development opportunities. This position is not just a job but a calling to make a substantial difference in the lives of many, fostering pathways to self-sufficiency and lasting change.

     

    Essential Duties and Responsibilities:    Other duties may be assigned.

    • Case Management: Oversee a caseload of individuals and families experiencing poverty, guiding them towards self-sufficiency through the Bridges of Self-Sufficiency program. –
    • Weekly Check-Ins: Conduct weekly check-in meetings via telephone with individuals and families to monitor progress and address immediate needs.
    • Monthly In-Person Meetings: Hold monthly check-in meetings in person, potentially including home visits, to establish a deeper connection and understanding of the participants' environments.
    • Mi-Bridges Navigation: Serve as a Mi-Bridges Navigator, assisting clients in navigating and accessing state benefit programs efficiently.
    • Professional Development: Become a certified Poverty Training Professional to enhance the mentoring and case management provided to clients.
    • Data Management: Diligently enter data into relevant databases, ensuring the accuracy of case notes, service descriptions, and financial documentation.
    • Database Inquiries: Handle inquiries from the EmpowOR database, providing prompt follow-up, resources, and referrals as necessary.
    • Communication: Maintain proactive and effective communication with participants, colleagues, funding sources, and the broader BOSS and Development Teams.
    • Resource Connection: Actively connect clients with additional resources within the agency and the community to overcome barriers to self-sufficiency.
    • Confidentiality* Uphold the integrity and confidentiality of all participant data and information. - Continuing Education: Participate in ongoing trainings and conferences to stay informed of best practices and developments in the field.
    • Enrollment Oversight: Ensure that participants are properly enrolled in the BOSS program and receiving the intended services.
    • Progress Monitoring: Utilize BOSS program measurement tools to regularly assess participant progress and determine necessary adjustments to their plan.
    • Training Documentation: Attend all required trainings and ensure accurate recording and documentation for future reference.
  • The position is home-office based with frequently required travel to serve a caseload of participants, in a multi-county service area. 
  • This position is temporary with an anticipated end date of May 2026.
