Responsibilities:
· Performs pre-charting for one (1) physician and assists this physician in the preparation of and submission of documentation and ensures that follow up documentation is submitted to referral sources and billing.
· Adheres to infection control in all aspects of patient care.
· Adheres to HIPAA and privacy rules and regulations.
· Is responsible for the administration of the CKD Clinics and ensures adherence to the CKD Standard Operating Procedures.
· Participates in the electronic tracking and follow up of new patient referrals and enters that the staff enters new patient information into the EMR system in accordance with Standard Operating Procedures.
· Ensures that patient telephone calls and communication with patients are adequate regarding items required for clinic visits, including, but not limited to completion of outstanding laboratory orders, diagnostic orders, insurance verification and insurance referrals.
· Supports the CKD clinic by performing vitals, medication reconciliation, verification of the existence of laboratory results and ensures each are accurately entered into the electronic health record.
· Complies with the CKD Standard Operating Procedures.
· Tracks patient scheduling of clinic visits within the EPIC system, in accordance with Standard Operating Procedures and participates in the confirmation of upcoming visits.
· Performs vitals, medication reconciliation, verification of the existence of laboratory results and ensures each are accurately entered into the electronic health record.
· Reduces lost to follow up patients by running monthly reports and following up with patients who miss appointments and reschedules patients when appropriate.
· Maintains clean and orderly examination rooms and work areas.
Job Type: Full-time
Pay: $18.00 - $21.00 per hour
