New Directions Treatment Services is seeking an experienced Community-Based Care Manager to join our team. This full-time position offers an opportunity to work with a dedicated non-profit organization committed to providing high-quality outpatient behavioral health services.
Job Description
The role of the Care Manager is to support the recovery process by coordinating and collaborating with community based services to assist with overcoming barriers to access and retention in treatment for individuals with Substance Use Disorders.
As a member of the Community Based Care Management Team, the Community Based Care Manager will facilitate referral and follow-up and provide assistance with scheduling and attending appointments to help those with Opioid Use Disorder navigate health care systems and social service providers within the community. The Care Manager will provide supports needed to engage and retain individuals with OUD in the treatment process. EOE
Key job duties:
- Complete and document an Individual Assessment of Needs for individuals in need of treatment for OUD
- Provide necessary referrals, schedule appointments, and provide transportation/access to necessary services identified in the Individual Assessment
- Collaborate with providers as necessary to ensure that services are meeting individual client needs.
- Provide face-to-face follow-up appointments, complete scheduled satisfaction surveys, and document outcomes.
- Provide telephone support to caseload during business hours
- Assist individuals to engage in and benefit from community services that help with recovery
Skills and Experience Required
- Proficiency in Microsoft Word and database management software
- High School Diploma
- Must have reliable transportation, Valid PA Driver’s license, and verification of State minimum insurance coverage
- Knowledge of community resources
- Knowledge of recovery and treatment for Substance Use Disorders
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
Expected hours: 37.5 per week
