Nutrition Care Coordinator
As a key member of the New Horizon Foods team, the successful candidate will be responsible for coordinating and implementing comprehensive nutrition care plans for seniors living in multiple locations.
Responsibilities:
- Develop and coordinate nutrition care plans for senior residents in multiple settings
- Assess the nutritional needs of residents and provide evidence-based recommendations
- Collaborate with healthcare professionals to ensure seamless care delivery
- Provide education and support to residents and their families on nutrition and healthy eating habits
- Monitor and evaluate the effectiveness of nutrition care plans
Qualifications:
- Proven experience as a Nutrition Care Coordinator or Dietitian
- Experience working with seniors in a senior living environment
- Strong knowledge of nutrition and healthy eating principles
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Bachelor's degree in Nutrition or related field
- Registered Dietitian (RD) certification
