We are seeking a dedicated and compassionate Outreach Success Coach to support individuals and families in achieving their goals. Using the Whole Family Approach and Family-Centered Coaching Principles, you will guide clients in securing housing, employment, and essential resources to improve their lives.
What You’ll Do
· Provide comprehensive coaching and case management through outreach by bringing services to clients where they are, in all weather conditions, to connect with individuals in their environments, especially those experiencing homelessness or living in areas with limited access to traditional services.
· Ensuring the connection of individuals to wrap-around services such as shelter, mental health support, substance use resources, and employment opportunities. This includes building trust with individuals in need, supporting their immediate survival needs, and assisting them in navigating long-term resources.
· Maintain accurate documentation and case notes in databases while participating in case conferencing.
· Collaborate with internal teams and funders to maximize positive client outcomes.
What You Need
Required
· Strong communication and customer service skills.
· Experience providing resources to families.
· Ability to make independent decisions based on funder and agency requirements.
· Valid driver’s license
Preferred
· Associate degree in Human Services or related field.
· Experience with workshop development.
· Background in social work, community outreach and event coordination.
